Save the Kids (STK) is a grass-roots nonprofit organization dedicated to establishing a movement that advocates for alternatives to incarceration that are peace oriented services for youth and the community, which helps to transform the juvenile justice system and end community and institutional violence.

There are three forms and one that is only needed for additional addresses. Two of the forms need to be sign in front of a notary. The other form needs to be signed and filled completely out not missing a year where the person lived. After all forms in this column are completed mail them off to:
